You’ll be prompted to request an access code, which will be sent to the mobile number you provided in your application. Enter the code to begin.

Yes. It’s important that you complete all questions truthfully and to the best of your knowledge as you have a duty to take reasonable care not to make a misrepresentation (please see the Product Disclosure Statement for full details). If you’re unsure how to answer something, please contact us or your financial adviser.
You can click the ‘Return incomplete’ button to send the application back to your adviser or contact us for support.
Most applicants complete the form in 15–30 minutes, depending on their medical and lifestyle history.
You may be asked to provide:
- height and weight;
- medical and family history;
- social habits;
- travel and hobbies; and
details of any medications, diagnoses, or treatments.
Please answer as accurately as you can. If you can’t recall a specific date, you may provide an estimated date or the month and year. If you do recall, tell your adviser as soon as possible to discuss and update.
Yes. Your progress is saved automatically. Use the same secure link in your email to resume your application at any time.
When applying for insurance, you must take reasonable care not to make a misrepresentation. Your answers must be honest, complete, and as accurate as you can reasonably recall so your application can be properly assessed.
If you do not take reasonable care not to make a misrepresentation, this can have serious impacts on your insurance (please see the Product Disclosure Statement for full details).
Your responses will be available to your financial adviser and our underwriting team for assessment.
Once you finish, the application will return to your adviser for final review and submission. After submission, we’ll send you a copy of your answers. If you notice any errors, please contact us immediately.
No. Completing the application does not activate cover. Interim Accident Cover begins only after your adviser submits the application.
